Mission Statement
Our mission is to support the community of Fulton County in attaining and maintaining health, by caring for its families in a holistic way without prejudice. This requires assessing the community's needs, then, providing education, service coordination and primary care when a need exists. All intervention is and shall be family centered with the overall goal being self care and self determination for the individual, the family, and the community. We believe that the foundation for "healthy" communities is "healthy" families.

Nursing Hours
Nursing hours provide an opportunity for residents to receive immunizations, have blood pressures taken or hearing tested. Pregnancy tests and family planning services and supplies are also available during nursing hours. Medical questions and/or concerns can be addressed by phone or in person during these hours.

Nursing hours are:
Monday, Wednesday and Friday:  8:30 am to 4:00 pm
1st Thursday of each month:  2:00 - 6:00 pm

Interpretation Services
Interpretation services are available to better serve Spanish speaking clients.
